- 1000 UAH denomination
- 2000 UAH denomination
- 5000 UAH denomination
Personalized gift certificate options (made to order):
- any denomination from 5000 UAH
- any service costing from 3000 UAH
Certificate payment options:
- cash or bank card at one of the DentArt network clinics;
- non-cash payment to bank account.
Certificate delivery options:
- in person at one of the DentArt network clinics;
- courier delivery in Kyiv or Odesa (delivery cost according to Uklon or Bolt taxi service rates);
- Nova Poshta (delivery cost according to carrier rates).

Gift certificate terms of use:
- Certificate validity period – 12 months from the date of purchase.
- Certificate cannot be returned or exchanged for cash.
- Certificate gives the right to purchase or receive any dental services and products at DentArt clinic network.
- Monetary certificate provides services for an amount equivalent to the card's denomination. The holder of such certificate can purchase one or more medical services within the certificate denomination limit.
- If the total amount of received services exceeds the certificate's nominal value, the certificate holder is obligated to pay the difference.
- In case the amount of received services is less than the certificate's nominal value, the difference in amounts is not compensated.
- Service certificate provides a specific service indicated on the card.
- To use the certificate, give the card to the clinic administrator when paying.
