Periodontal chart: what is it and what data does it contain?

In fact, it is a medical document that provides comprehensive information on the condition of the patient's periodontal tissues. Whereas in the past, periodontal records were mainly kept in paper form, it is now a digital document that is automatically created in special software on a computer. The results of the diagnosis are entered directly into the software and its database, eliminating the possibility of any errors and speeding up the process of interpreting the diagnostic data obtained.

The periodontal chart contains the following information required by the periodontist:

  1. A general verdict on the current state of periodontal tissue health, including the diagnosis (healthy, pockets of gingivitis, or some degree of periodontal disease).
  2. Specific diagnostic data for each individual tooth, namely information about:
    • the presence of plaque;
    • the depth of the periodontal pockets;
    • bleeding gums and/or a discharge of pus;
    • mobility of teeth and the degree of its severity;
    • the state of bone tissue, etc.

Why is a periodontal patient card needed?

Thanks to this document, all information about the periodontal condition is conveniently structured and systematised. Accordingly, our dentist-periodontist can almost instantly refresh data on a particular patient's periodontal disease, previous examinations and prescribed treatment.

The periodontal chart also helps the doctor with an individual treatment plan and prognosis for each particular tooth. It is also convenient to use this document to monitor the periodontal condition of a particular patient over time, as it records all the results of examinations, both first-time and repeated. The dynamics of the changes make it possible to assess the effectiveness of the treatment, to detect a new wave of pathology progression or to confirm its attenuation (remission).

The periodontal chart is also useful for the patient. It is a clear and quite simple way of demonstrating existing problems with the periodontal tissues. A periodontal patient chart can motivate adherence to treatment recommendations. Periodontitis itself doesn't cause any pain, but that doesn't mean there isn't a problem. The periodontal chart clearly demonstrates the existing problem areas, shows how the size of periodontal pockets has changed since the start of therapy, which convinces to continue treatment.
