Sinus lifting is a type of osteoplasty (bone grafting) that affects the sides of the upper jaw. This surgery is one of the preparatory steps before implantation and is required when it is necessary to compensate for bone deficiencies that prevent the implant from being securely placed.

Indications and contraindications for sinus lifts

Such surgery may be necessary when implants are placed in the lateral sections of the upper jaw. The indications include the following:

  • bone atrophy;
  • anomalies in the structure of the maxillary sinuses (i.e. maxillary sinuses);
  • age-related jaw changes.

Not all cases require such a procedure before implantation. If there is enough bone volume, surgery is not necessary, and in practice there are even more such cases. But sometimes implantation is impossible without sinus lifting. It will probably be necessary if:

  • your tooth has been missing for a long time and an implant has not been put in its place straight away: during this time, the bone thins considerably;
  • infection of the bone tissue has occurred, which usually affects bone volume and density;
  • there has been trauma and/or a traumatic extraction of a tooth resulting in damage to the structure of the upper jaw.

Contraindications for sinus lifting are:

  • problems with blood clotting;
  • the presence of pathologies of the oral cavity;
  • mental disorders;
  • intolerance to anesthesia.

Why is a sinus lift necessary?

A reliable implant that will last for many years should be at least 10 mm long. Consequently, a sufficient amount of bone material is needed for a good implantation. In the area of the sinuses (on the sides above the upper jaw), the tissues can be thinned. One of the reasons is a long-standing tooth loss. Attempting to insert the implant in this situation will cause it to enter the maxillary sinus, and complications and rejection will ensue. To prevent this from happening, you need special preparation for implantation.

Types of sinus lifting

The choice of a suitable intervention at our clinic is preceded by a careful examination of the patient's oral cavity and a CT scan, i.e. a CT scan of the upper jaw. The resulting 3D images allow doctors to study the maxillary sinus, determine the amount of bone material that needs to be compensated, and/or even run a virtual simulation of the procedure.

There are two main types of procedures, namely open and closed:

The closed sinus lifting
Рrocedure is slightly simpler and less traumatic. This procedure is usually used when 2-3 mm of bone tissue is insufficient for a good implant placement.

Open sinus lifting
Surgery with "gingival opening" is used in all other situations, i.e. when there is severe thinning and a lot of bone deficit needs to be compensated for.

Stages of the sinus-lifting operation

The course of the procedure depends on the type of intervention. Open sinus lifting includes:

  • peeling of the oral mucosa (”opening" of the gum);
  • bone perforation: making a hole with a special cutter;
  • peeling of the shell of the maxillary sinus cavity - Schneider's membrane, as a result of which the bottom of the sinus rises;
  • filling of the formed cavity with special osteoplastic material and its integration with the bone;
  • covering the bone with a barrier membrane and suturing the gums.

In this case, a one-stage implantation is not performed: the implant is placed 4-6 months after the procedure is completed.

A closed sinus lift is considered a gentler procedure. Instead of an open access, a small "window" to the maxillary sinus is formed and osteoplastic material is injected into the sinus through this window. The main stages are as follows:

  • forming an access without complete bone perforation;
  • elevation of the floor of the maxillary sinus;
  • laying of osteoplastic material.

Often an implant is placed immediately, which is also one of the distinguishing features and advantages of the closed procedure.

Postoperative period: duration and recommendations

The total recovery period usually does not exceed 2 weeks. Post-operative follow-up is necessary until the stitches have been removed, and this procedure is usually carried out at our follow-up visit.

Key recommendations during the recovery period include the following:

  • it is not advisable to eat before the anaesthesia has worn off - usually for the first 2 hours after the manipulation;
  • for the first time, to reduce swelling and inflammation, we recommend applying compresses to the wound - for about 3 hours, alternating between 20 minutes of holding the compress and 20 minutes of resting;
  • until the wound has healed, you should not wash or try to clean it, or use hard, very hot or spicy food. A diet of soft and liquid foods should be followed for up to 7-10 days;
  • after eating and brushing your teeth, it is advisable to keep antiseptic solutions in your mouth (not to rinse);
  • do not play wind instruments or inflate your cheeks during rehabilitation. Also, you should cough and sneeze with your mouth open - for up to 2 weeks.

Are complications possible after sinus lifting?

Individual complications are theoretically possible directly during manipulation. Thus, a rupture of the Schneider's membrane can occur. In this case, the doctor stops the operation and takes measures that allow the membrane to be repaired. If its damage is minimal, the sinus floor elevation procedure continues once the rupture has been repaired.
