Is it always necessary to remove wisdom teeth?

Wisdom teeth, aka third molars, are considered unnecessary in many European countries. That is, they have completely lost their primary function in the course of evolution - their direct involvement in chewing. But it is customary to get rid of these teeth only if there are the right indications.

For example, if the third molars have erupted normally, are in exactly the right place, do not interfere with neighbouring teeth, and are properly hygienised to protect them from various dental problems, they can be preserved. But such situations are quite rare. Most patients with our doctors in Kyiv have indications for surgery.

When should wisdom teeth be removed?

The different types of wisdom teeth (fully grown, not erupted (retinitis) and/or closed by the "hood") have their own indications for removal.

Fully grown third molars are disposed of if:

  • it is difficult to get close to them for cleaning, which causes caries and/or gum inflammation;
  • they cause the neighbouring second molars to "suffer";
  • the positioning of these teeth causes trauma to the tongue or cheek (including chronic biting);
  • the incorrect position causes deformation of the entire tooth row;
  • crunching and/or pain in the temporomandibular joint occurs when opening the mouth or chewing.

Unerupted wisdom teeth are removed if:

  • in the X-ray, our doctors see that the third molars are resting on the neighbouring teeth and can damage them, which in turn means that they also have to be removed (and these are our most important chewing teeth);
  • they put pressure on the "sevens", causing root caries;
  • implantation is performed. Wisdom teeth should be removed so that they do not damage the implant in the future;
  • also for orthodontic indications, when correcting the bite.

If the eighth tooth erupts with gradual tearing of the mucosa and the formation of a "hood", the first thing to do is to remove the growth to open up access for normal hygiene. If this is not done, the third molar itself will soon have to be removed.

If there is an indication of getting rid of one eighth tooth, our doctors recommend removing two on the upper jaw and two on the lower jaw at once. When a pair of upper teeth are left behind, there is a sinking of the teeth because they have lost their lower neighbours. This increases the risk of crooked teeth, problems with the temporomandibular joint (clicking and pain in the joint) and other complications.

Simple and complex removal of wisdom teeth in DENTART clinic

Our surgical doctors perform simple and complex wisdom teeth extractions. There are usually fewer problems with the upper ones, as the bone tissue here is not as dense. In addition, the roots of the lower teeth are often more branched, making the operation more time-consuming and the recovery process longer.

The simple procedure is performed without incising the gums and/or drilling out the bone. The total duration is the same as with other teeth that are closer and easily accessible. Our doctor will treat the wound where the tooth was extracted, stitch it up and tell you how to take care of it at home.

Complex extraction is characteristic of the lower jaw, namely wisdom teeth that have not fully erupted or have grown horizontally. Since the tooth is located in the bone or under the gum, it is necessary to perform minimal incisions. And then, after extraction, stitch up. If a long procedure is envisaged, you can consider options for anti-stress treatment in your sleep together with our doctor.
