Two-stage dental implantation is a classic restoration procedure. It is used for any number of lost teeth, up to full adentia, and involves a pause (3 to 6 months) between the implant being placed in the jaw and the fixed denture being fitted. This method is considered the most predictable and the safest, as the risk of rejection of the artificial root is extremely low (less than 1%).

How is a two-stage implantation performed?
The following treatment steps are foreseen for two-stage implantation:
- Preparatory stage with a general examination of the patient. A computerized tomography is performed and the jaw and jaw system is examined. Blood samples are also taken. Oral hygiene is performed, including treatment of gums, tooth decay, and removal of tartar. Implants are also chosen at this stage.
- Bone augmentation (if necessary). If the amount of bone is reduced (atrophy), osteoplastic surgery may be required, including a sinus elevator. Bone grafting is most often required in situations where the patient decides to restore a lost tooth, not immediately, but over time. This causes atrophy, and the amount of bone becomes insufficient to support an artificial root.
- Implantation itself. An artificial root is implanted in the jawbone, and the stitches are applied.
- Gum Former.After the implant is fully engrafted (after 3-6 months), the gums are exposed, and a guide is inserted for 7-14 days, to create the correct contour of the gums around the future crown.
- Prosthetics begins with the placement of a special connecting element (abutment) between the implant and the prosthetic structure.
- The final stage is placement of the crown.

When is conventional (two-stage) implantation preferable?
Two-stage implantation offers a high level of reliability and predictable outcomes. It is therefore recommended in the following situations:
- 1. Need to replace several teeth at once.
- 2. Restoring teeth that bear the greatest chewing loads, especially the eight primary molars (the main chewing teeth).
- 3. Significant bone deficiency requiring sinus lifting or other bone grafting procedures to augment tissues.
- 4. Chronic inflammatory conditions of the gums or other oral tissues.
- 5. Complex tooth extraction that makes immediate implantation impossible.
- 6. Other contraindications and/or elevated complication risks that limit alternative protocols.
- 7. Previous unsuccessful implant attempts (e.g., cases of implant failure/rejection).

How many stages are there in conventional implantation?
There are two main stages: the surgical stage (implant placement), and the prosthetic stage (crown placement). You can also distinguish a “stage zero”—preparation—which always includes diagnostics and may involve therapeutic treatment (e.g., caries management) and/or bone grafting procedures to augment the bone.

How long after implant placement is the crown installed?
In the classic protocol, the crown is placed 3–6 months after the implant surgery. The exact interval is individualized, since the time required for osseointegration depends on factors such as patient age, bone volume and density, harmful habits, coexisting conditions, and the implant system used, among others.
