How is all-on-4 implantation done at DENTART?

Our implantologists pay increased attention to preparation and thorough diagnostics because no successful dental restoration is guaranteed without them. The basic steps of All-on-4 implantation include:

  • initial consultation with an implantologist at our clinic;
  • CT scanning of the jaw;
  • oral cavity sanation and extraction of the remaining severely damaged teeth;
  • surgical procedure with implantation through a puncture hole;
  • placement of a temporary denture performed on the day of the surgical phase (sometimes a few days later);
  • fixation of permanent dentures. It is carried out after the full engraftment of artificial roots (in 5-6 months).

Prosthetic options for All-on-4 implantation

The All-on-4 method provides rapid dental arch restoration through the placement of a prosthesis on four implants. Most commonly, immediate-load implantation is used, where a fixed prosthesis is attached immediately after implant placement. This allows the patient to quickly return to their normal lifestyle.

In some cases, a two-stage approach may be applied. First, the implants are placed, and after several days, a temporary prosthesis is installed. This method is used when additional time is needed for tissue adaptation and implant stabilization.

In our clinic, All-on-4 implantation is performed using the following methods:

  1. Immediate-load. Implants with immediate loading are placed right after tooth extraction.
  2. Guided surgery. Using digital 3D scanning, a custom surgical guide (template) with four holes for implants is created. It is fixed in the oral cavity and allows for implant placement at the required angle.
  3. Classical method. The surgeon makes an incision and reflects the gingival flap, as well as performs other necessary manipulations to place four implants.

The final implantation option is determined after consultation and diagnostics, taking into account the condition of the bone tissue and individual characteristics of the patient.

What are the benefits of all-on-4 implantation?

All-on-4 concept means four implants in one jaw in a special pattern. Two of them are placed instead of the front incisors vertically and two more are plugged in on the sides but at a 45-degree angle. Then special inclined abutments are placed on the latter providing a strong and secure fixation of the prosthesis.

Such dental implantation has its advantages:

  1. Cost-effectiveness. Only 4 implants are used to restore your teeth.
  2. Implantation in one day. More often than not, immediate loading is recommended, so a temporary denture is placed right after the implanting itself.
  3. Bone grafting is not necessary. Using tilted implants means no osteoplasty is necessary, even if there are slight jawbone deficits around the occlusal teeth.

But there’s one important point. All-on-4 implantation is not possible without an excellent condition of the jawbone. As practice shows, sometimes alternative concepts for dental implants such as All on six are a more reliable and justified solution.

Recommendations and contraindications for the all-on-4 concept

All-on-4 implants can be used in the situations of:

  1. absence of all teeth on the jaw (total adentia);
  2. partial adentia with poor condition of the remaining teeth;
  3. good bone condition that makes it possible to avoid more expensive reconstruction methods;
  4. contraindications to bone grafting operations.

Relative contraindications are systemic diseases such as:

  • chronic liver or kidney pathologies;
  • blood clotting disorders;
  • serious cardiovascular diseases;
  • diabetes mellitus, etc.

All-on-4 dental implantation is not recommended in cases of serious jaw bone atrophy (medium and severe form). In these cases, one should use All-on-six concept or other restoration methods for a secure implants fixation.

  • What advantages does All-on-4 have over removable dentures?

    The key benefits are comfort and quality of life. With All-on-4, there’s nothing to take out at night, no anxiety about a denture loosening, shifting, or falling out. Functionally and aesthetically, implants outperform removable dentures: your smile looks natural, and you can chew a wide variety of foods—including harder textures. Plus, All-on-4 helps prevent ongoing jawbone atrophy and supports healthy gingival circulation.

  • When is All-on-4 not an option?

    All-on-4 is generally not used in cases of moderate to severe jawbone atrophy. In such situations, alternative solutions—such as All-on-6—are recommended. The “all-on-four” concept is also contraindicated when there are absolute bans on implant surgery in general (e.g., uncontrolled diabetes, serious cardiovascular conditions, or coagulation disorders).

  • How is All-on-4 placed?

    At DENTART in Kyiv, All-on-4 can be performed immediately (same-day), via a conventional (flap) approach, or using a surgical guide. In immediate cases, implants are inserted directly into the sockets created right after extractions. The classic protocol involves a surgical incision with flap reflection to complete all necessary steps. With a surgical guide, All-on-4 implants are placed through small punctures; the custom-made guide (tray/template) in the mouth indicates the exact entry points and angulation for drilling.
